The Athletic Training Staff at City College of San Francisco is responsible for the prevention and care of athletics injuries for the student-athlete. The Athletic Training Staff is committed to providing exceptional services to the student-athlete by utilizing a highly qualified and motivated staff which includes two certified Athletic Trainers, a team physician, team orthopedic surgeons and a variety of allied specialists.
CCSF Returning and Incoming Student-Athletes, please follow the directions below to prepare for your medical clearance:
Please make sure all your documents on healthy roster are completed and up to date. All physicals are valid for 1 year from the date of exam. If your physical examination is expiring this summer, use the following link to sign up for on-campus physicals on July 28, 2022 -OR- use the attached template to bring to your personal physician for a physical. Reminder that examinations must be signed off by a MD (medical doctor) or DO (doctor of osteopathy) and should include a stamp or business card.

Before you can sign up for a physical you must create a Healthy Roster Account and complete all forms.

Please allow 24 hours for the invitation to be sent. The invite will have a code specific to each individual as well as a link to download the app on your phone or tablet. Once you've logged in, navigate to your profile and click the documents section. Complete ALL of the required 2022-2023 documents:
- Assumption of Risk
- Consent to Treat
- Covid-19 History Questionnaire
- Insurance Information
- Medical History Questionnaire
- Student-Athlete Emergency Contact Form
- Covid-19 Vaccination Card or Exemption (upload a picture or PDF document)

If you decide to go to your personal physician, print and bring the attached forms to your doctor's visit for them to sign. Once completed, you can upload the forms as document types "Medical History Questionnaire" and "Physical Evaluation."
ALL PHYSICAL EXAMS MUST BE SIGNED OFF BY A MEDICAL DOCTOR (MD) OR DOCTOR OF OSTEOPATHY (DO) WITH AN OFFICE STAMP OR BUSINESS CARD.
